Participates in care planning, and addresses various medical needs.
Assesses and manages pain and other symptoms related to the disease to ensure comfortable care.
Provides psychosocial support to patients and their family, and family. Coordinates patient care with physician, hospice team and community resources.
Actively manages the day-to-day patient care plan and process.
Provides assistance with activities of daily living and personal care.
Provides spiritual support and comfort to patient and/or their family.
